K-8th PROCESS FOR RETURNING IN 2020-21:

Use these forms for all returning students entering Kindergarten through 8th Grades.

 

Re-Enrollment Process:

We are thrilled that you will be rejoining us in the 2020-21 school year!  Please follow all steps below:

Failure to complete your forms by the due dates could result in a loss of placement at SMCS. 


step 1- Intent-to-return forms:

Complete this online form between March 2nd & March 16th, 2020*

This form tells us if you intend to return for 2020-21, and informs how many spaces we give away in the lottery. All Intent-to-Return Forms are due by 3:00 pm, MST, on March 16th.

*Please note: If you are interested in applying for Early Access to 1st Grade, your Application Portfolio must be received by January 7th, 2020. Visit the Gifted and Talented page for detailed information.

STEP 2- Supply Fee*:

Due May 13th, 2020

The amount owed for each K-8th student for the 2020-21 school year is TBD. We ask for this fee in lieu of asking you to buy supplies.

*If this presents a hardship for your family, please contact the SMCS Registrar, Courtney Landry at courtney_landry@salidamcs.org. Supply fees are non-refundable.


Step 3- Re-Enrollment Forms**:

Complete between May 1st and May 15th, 2020

This form is your actual re-enrollment form and is needed every year. As a school of choice, you must indicate that you are choosing us, by re-enrolling your student(s). Failure to submit this form by May 15th, 2020 will result in loss of placement at SMCS.

STEP 4- Health Appraisal & Immunization Forms

Due by August 13th, 2020

Health Appraisal, Immunization Forms, & Copy of Insurance Card Due by August 13th, 2020. Please download the Health Appraisal Form and take it to the doctor, if your child is due for a visit. The top portion must be filled out by a parent and returned, even if your child is not due for a physical. Also, you will need to submit a copy of your child’s Immunization Records or an Exemption Form and a copy of their Insurance Card.

You will not be permitted to attend school until these forms are received. If you need to schedule your appointment later, due to insurance timing, please notify the office of your appointment date.
